Form 10A – New Chapter Member

When new members receive their initiatory and/or DeMolay degree in the Jurisdiction of Michigan, you submit a FORM-10A , and send a check (or money order) in the amount of $50 payable to Michigan DeMolay to Dad D’Aguanno. (breakdown = $15 for Michigan DeMolay + $35 for DI).

DO NOT send any money to DI or complete to Form-10 on the DI web site.

When Dad D’Aguanno receives the FORM-10A and the $50 check (or money order), he will complete the actual FORM-10 on the DI web-site through E-Scribe for the young man.
